We assist with the safe administration and supervision of medications, ensuring that doses are taken correctly and on time. Our nurses also provide medication education and help clients understand their prescriptions and side effects.
We provide ongoing support for catheter maintenance and continence management. Our nurses monitor for signs of infection, ensure hygiene is maintained, and educate clients on daily self-care routines.
Our nurses assist with blood glucose monitoring, insulin administration, and health education to promote better self-management. We help clients understand dietary needs, foot care, and lifestyle factors that affect their condition.
We support clients with feeding tube management, including site care, hygiene, and troubleshooting equipment. Our nurses provide training for carers and family members to ensure safe feeding practices and nutritional monitoring.
Our team assists with stoma bag changes, skin care, and stoma site maintenance to ensure comfort and prevent infection. We empower clients with knowledge and confidence to manage their stoma independently when possible.

Services are tailored to each client’s physical, emotional, and social needs.
Our services are available to participants under the NDIS, recipients of Home Care Packages, and eligible DVA clients with a valid referral or approval. We also accept private clients seeking short- or long-term nursing care at home.
You can contact Specific Care directly, or your Support Coordinator, GP, or hospital discharge planner can send a referral. Once we receive your details, our clinical nurse will conduct an initial assessment to create a personalised care plan that fits your needs and funding requirements.
During your assessment, our nurse will review your medical history, current medications, ongoing health needs, and environment. This helps us create a care plan that ensures safety, continuity of care, and alignment with your goals and clinical priorities.
Yes. All our nurses are AHPRA-registered (Registered or Enrolled Nurses) and have current police checks, NDIS Worker Screening, CPR and First Aid certificates, and infection control training. Specific Care also maintains professional indemnity and public liability insurance.
Visit frequency depends on your individual care plan and health needs. Some clients require daily or weekly visits, while others may only need periodic assessments or wound reviews. We adjust your schedule as your condition changes.
For NDIS participants and DVA clients, services are billed directly under your approved plan or referral — there are no out-of-pocket costs. Home Care Package clients can use their allocated funds, while private clients are provided with a clear quote before services begin.
